How the WV One-Visit Process Works

Submit a copy of your government-issued ID and vaccination records.

Complete required laboratory testing (if needed).

Once all laboratory results and vaccine documentation are received, your form I-693 will be prepared and finalized at your office visit.

• Labs and vaccines may be covered by most insurance plans. Patients without insurance may complete required labs testing through a local laboratory such as LabCorp.
• Laboratory and vaccine documentation must be received prior to final completion of Form I-693.
• In-office visit and signature are completed once all required results are available.
• Payment is required to secure your appointment.
• Appointments are scheduled based on completed intake and availability.

Completion typically takes about 2-3 weeks after all required results are received.
