Form I-693 is the USCIS Immigration Medical Examination required for applicants applying for Adjustment of Status (Green Card) in the United States. The exam must be completed by a USCIS-designated Civil Surgeon and submitted to USCIS in a sealed envelope.

You need Form I-693 if you are: • Applying for Adjustment of Status (Form I-485) • Filing your Green Card application from within the U.S. • Requested by USCIS to submit or update your medical exam

The I-693 Express process begins with virtual intake and physician review, followed by completion of any required labs and vaccines. Once all requirements are met, eligible applicants complete the in-office exam and Form I-693 is finalized as appropriate.

Required bloodwork for Form I-693 is ordered as part of the process and is billed directly by the laboratory. For patients with health insurance, required I-693 bloodwork is typically covered when completed through insurance, as these labs are considered preventive services. Coverage depends on the individual plan. Required vaccines are available in the office for patients without health insurance. Patients with health insurance are typically directed to complete required vaccines through a pharmacy or healthcare provider prior to the in-office exam. In-office vaccines for uninsured patients are offered at a reduced self-pay cost

Virtual visits are scheduled after payment and intake completion as part of the Express review process.

Same-day sealed Form I693 is available only when all required labs, vaccines and documentation are completed in advance and Express requirement are met.

The I-693 exam fee is self-pay and covers physician services and completion of Form I-693. Laboratory tests and vaccines may be completed through insurance or self-pay, depending on the service.
